WAIVER FORM - STAND UP PADDLE (SUP) ACTIVITIES
Stand up paddle (SUP) is a physically challenging water sport activity and carries with it inherent risks that we cannot entirely eliminate. Those risks can result in serious injury to you.
Inherent Risks
Rules
· You must abide by current UK Health and Safety advice.
· You must be able to swim 50 meters (160 feet).
· You do not have any medical or physical conditions that would affect your participation in SUP.
· You must act responsibly and sensibly at all times.
· You must not participate in SUP if you are under the influence of alcohol or non-prescription drugs.
· You must follow any safety warnings or instructions displayed or given to you by your SUP instructor.
· We are not qualified to express an opinion that you are fit to safely participate. You must obtain professional or specialist advice from your doctor before participating.
· You must advise the instructor if you suffer with any of the following medical conditions:
(i) Asthma
(ii) Previous broken bones/dislocated joints
(iii) Diabetes
(iv) Wearing contact lenses/hearing aids
(v) Allergies
(vi) Any disabilities
(vii) Any other medical conditions
Liability
For your protection, we will strictly enforce all UK Health and Safety advice, including but not limited to COVID-19 guidance.
In the absence of any negligence or other breach of duty by us, participation in SUP is entirely at your risk.
Loss or damage to property
We do not recommend that you take any personal, or valuable items with you during SUP activities.
In the absence of any negligence or other breach of duty by us, we are not responsible for any theft, damage, destruction or loss of your property or belongings during SUP activities.
IT IS IMPORTANT THAT YOU READ AND UNDERSTAND THIS STATEMENT BEFORE SIGNING. IF THERE IS ANY TERM THAT YOU DO NOT UNDERSTAND, THEN PLEASE DISCUSS IT WITH US BEFORE SIGNING.
